Healthcare Programs
Covered California
Covered California is a program made possible by the Affordable Care Act and provides low-cost and subsidized health insurance to all Californians who would not otherwise have healthcare insurance.
Medi-Cal
Medi-Cal is California's Medicaid health care program. This program pays for a variety of medical services for children and adults with limited income and resources.
Cal-Fresh
CalFresh is for people with low-income who meet federal income eligibility rules and want to add to their budget to put healthy and nutritious food on the table.
Other Assistance Programs
340B Prescription Program
As a patient of Bay Area Community Health, you are eligible for a discount on prescribed medications that can save you 20% to 50% on the cost of your medication.
We have contracted with a national pharmacy chain and a local independent pharmacy to offer convenient locations for your prescriptions to be filled and picked-up.
Your Primary Care Physician can send an electronic prescription request to its contracted pharmacies or give you a hand-written prescription for you to take to the contracted pharmacy.
CHDP Program
Gateway provides immediate comprehensive services to potentially Medi-Cal eligible children under 18 years old at no cost on the basis of “presumptive eligibility” until such time that it is determined that they don’t meet the qualifications for Medi-Cal or until they are provided Medi-Cal benefits.
The CHDP program provides complete health assessments for the early detection and prevention of disease and disabilities for low-income children and youth. A health assessment consists of a health history, physical examination, developmental assessment, nutritional assessment, dental assessment, vision and hearing tests, a tuberculin test, laboratory tests, immunizations, health education/anticipatory guidance, and referral for any needed diagnosis and treatment.
The CHDP program oversees the screening and follow-up components of the federally mandated Early and Periodic Screening, Diagnosis, and Treatment (EPSDT) program for Medi-Cal eligible children and youth.
Comprehensive Perinatal Service Program (CPSP)
The program offers a wide range of services to pregnant Medi-Cal recipients from the date of conception through 60 days after the month of delivery. Services include: nutrition counseling, psychosocial counseling, mental health counseling, health counseling (including home visits), and health education programs (individual and groups).
Every Woman Counts (EWC)
EWC is a comprehensive, public health program that assists uninsured and underinsured individuals in obtaining high-quality cancer screening and follow-up service.
Family PACT (Planning, Access, Care, Treatment)
Family PACT services are designed to support contraceptive methods for women and men, as gender appropriate, by assisting individuals who have a medical necessity for family planning services.
Family PACT includes assistance with family planning-related services to achieve and maintain optimal reproductive health.
There are two categories of services available in the program: Family planning services and family planning-related services for specified reproductive health conditions.
Health Homes Program (HHP)
The Health Homes Program (HHP) is designed to serve eligible Medi-Cal beneficiaries with complex medical needs and chronic conditions who may benefit from enhanced care management and coordination. The HHP coordinates the full range of physical health, behavioral health, and community-based long-term services and supports (LTSS) needed by eligible beneficiaries.
In addition to offering screening and diagnostic services, the program is designed to facilitate regular rescreening of women with normal or benign breast and/or cervical conditions to provide diagnostic services for individuals presented with symptoms and/or abnormal screening results and to refer for treatment when necessary.
