Health Information Management (HIM)

What We Do

The HIM team, is responsible for maintaining, protecting, and managing your health records. This includes documentation of your medical and dental visits, test results, immunizations, and other healthcare services you receive at Bay Area Community Health.

We ensure your information is:

  • Confidential – Only accessible to you and authorized parties

  • Accurate – Carefully recorded and maintained

  • Available – Ready when you need it for care, insurance, or legal purposes


Who We Help

We support:

  • Patients requesting access to their own medical or dental records

  • Parents and legal guardians accessing their child’s records

  • Healthcare providers coordinating your care

  • Insurance companies (with proper patient consent)

  • Attorneys or legal representatives with valid authorization


How to Request Medical Records

Irvington Location

  • By Appointment Only: Call 510-252-5848 to schedule a time to pick up your records.

  • Patients may also visit Irvington to complete a Release of Information (ROI) form and leave it with front desk staff. Our HIM team will collect and process all forms.

Aborn Location

  • Walk-in service is available Monday–Friday, 8:00 AM to 4:30 PM.

Please bring a valid photo ID when picking up records. Depending on the request, processing times may vary.
