Program Spotlight: Maternal Care Access and Equity Project

Bay Area Community Health is helping improve maternal and infant health outcomes through its Maternal Care Access and Equity Project, a pilot program designed to support underserved families in East San Jose.

The program was developed in response to persistent maternal health disparities and provides care coordination, transportation assistance, prenatal and postpartum supplies, referrals, and educational workshops focused on nutrition, lactation, parenting, and maternal wellness. Participants receive support throughout pregnancy and for up to one year after delivery.

A unique feature of the program is its cohort-based model, which brings expectant and new mothers together in a supportive environment where they can learn, share experiences, and build lasting connections. These relationships help reduce isolation and encourage healthier outcomes for both mothers and babies.

From September 1, 2025, through August 31, 2026, the Stanford Maternal Care Access and Equity Project served 382 unduplicated mothers and provided 11,295 encounters, demonstrating strong engagement and ongoing access to care. Through personalized support and regular follow-up, participants were connected to essential health services and community resources that promote healthy pregnancies, safe deliveries, and positive postpartum experiences.

“Every mother deserves to feel supported, informed, and empowered throughout pregnancy and beyond,” said Pheakdey Linde Chea, Program Manager of Women’s Health. “Through strong partnerships and community-centered care, we are helping mothers and babies access the resources they need for a healthy start.”

By addressing barriers to care and connecting families with critical services, Bay Area Community Health continues its commitment to advancing health equity and ensuring that mothers and babies have the support they need to thrive.
