Diabetes Prevention Awareness: Addressing the Decline in Diagnosis Visits through Community Education and Health Screenings 

According to our recent Uniform Data System (UDS) data, Bay Area Community Health (BACH) has observed a notable decline in diabetes diagnosis visits throughout 2023. This reduction is likely due to limited access to care, fewer individuals seeking medical assistance, and insufficient awareness of the need for regular health screenings. Left unaddressed, these factors could increase health risks for vulnerable community members, leading to complications and impacting overall wellness. 

In response, BACH is committed to enhancing community education around diabetes prevention and reinforcing the importance of regular wellness checks. Our mission is clear: to raise awareness, increase access to health screenings, and empower individuals to take proactive steps in managing their health. 

Community Engagement and Outreach 

Eder Torralba, our Outreach Program Manager at BACH, leads our outreach initiatives and plays a pivotal role in our diabetes prevention strategy. Understanding the unique needs of our community, Eder and the outreach team develop health education programs that are both accessible and relevant. 

Through strategic partnerships with local organizations like Santa Clara Family Health Plan and Tri-City Volunteers Food Bank, our outreach team ensures that BACH effectively connects with underserved populations, providing them with essential information, support, and resources to maintain their health. 

Additionally, our outreach team coordinates wellness checks and diabetes screenings at convenient community locations, making critical health services more accessible. These initiatives are designed not only to promote early diabetes detection but also to empower individuals with the knowledge and resources needed to make informed health decisions. 

Outreach Events and Education Initiatives 

In 2024, BACH has taken a proactive approach to diabetes prevention through a variety of community events, led by Eder and the outreach team: 

  • Screenings and Fairs: Over 3,071 community members participated in health screenings at local health fairs and clinics, aiding in early diabetes detection and follow-up care. 

  • Educational Sessions: Health education sessions, reaching 8,392 attendees, focused on raising awareness around diabetes prevention and lifestyle modifications, helping improve community health literacy. 

 

Projected Outcomes and Future Goals  

As we continue promoting diabetes awareness and encouraging regular screenings, we expect to see an increase in diagnosis visits and improved early detection. This progress is essential for reducing the long-term impact of diabetes and associated health risks. 

Looking toward 2025, BACH has set specific objectives to build on our current efforts: 

  • Increased Screening Rate: With a goal to boost diabetes screenings by 25% over 2024, we’re expanding outreach efforts to connect with high-risk populations and improve early detection rates. 

  • Enhanced Education Outreach: Through new workshops and educational materials, we plan to reach an additional 1,000 community members, focusing on diabetes prevention and self-management. 

  • Improved Follow-Up Care: Our aim is for 80% of individuals diagnosed during screenings to receive timely follow-up care and personalized management plans, promoting healthier outcomes and quality of life. 

 

BACH is dedicated to advancing diabetes prevention and management within our community. Through our outreach initiatives, partnerships, and robust education initiatives, we’re fostering a stronger foundation for community health. By encouraging awareness, expanding access to screenings, and advocating for proactive health management, we support everyone in leading a healthier, more informed life.
