Diabetes Education in South Santa Clara County: A New Initiative Launching in March

Bay Area Community Health (BACH) is launching a Diabetes Education Program in South Santa Clara County, funded by the AWS Grant. This initiative aims to provide essential education and support to individuals managing diabetes, equipping them with the knowledge and tools needed for a healthier life.

The first class is scheduled for March 2025, marking the start of a comprehensive program addressing the rising prevalence of diabetes in the region. Participants will learn about nutrition, physical activity, medication management, and lifestyle changes to help them take control of their health. To ensure accessibility, the program will include culturally responsive materials and bilingual support for diverse community members.

Addressing Health Disparities

Diabetes remains a significant public health concern, particularly in underserved communities where access to preventive care and disease management is limited. This program seeks to bridge that gap by offering free educational workshops, hands-on demonstrations, and one-on-one guidance from healthcare professionals.

In Santa Clara County, Latinos make up 25% of the population, a percentage expected to remain stable through 2060. In Gilroy, located in South Santa Clara County, Latinos account for 58% of residents, highlighting the need for targeted health interventions.

Latinos in the county experience higher rates of obesity and diabetes compared to other groups. 33% of Latino adults are classified as obese, and the rate of diabetes-related emergency room visits among Latino adults is 445.9 per 100,000, more than double the rate for White non-Hispanic adults (199.8 per 100,000) (Santa Clara County Public Health).

By prioritizing culturally relevant education and accessibility, this program takes a crucial step toward reducing diabetes-related health disparities and empowering the South Santa Clara County community.
