Sliding Scale

Bay Area Community Health welcomes most insurances, private pay, Medi-Cal and Medicare. For patients without insurance or underinsured, we offer a sliding fee scale based on income and family size to ensure that quality compassionate health care is readily accessible to everyone.

Sliding Fee Scale

Our sliding fee scale offers significant discounts to individuals and families who qualify on the basis of limited income and / or family size. This program allows qualifying patients to receive medical, dental care, and optometry care at a lower cost. Eligibility for this program must be renewed annually. Please bring at least one of the following forms of documentation with you if you wish to apply:

  • Your most recent tax form (1040, 1040A, 1040EZ)

  • If you do not file taxes, a recent check stub(s) from your employer

  • A summary of benefits letter from SSA, welfare, or social services agency

Lab & Pharmacy Charges

Lab and pharmacy charges may have a co-payment, a service charge, a sliding discount, or a share of the cost, all of which are required at the time services are rendered. Lab and pharmacy charges may represent additional charges that may be billed separately, depending on the patient’s insurance/program.

Patients should always bring insurance cards, health program membership cards, and proper identification to every visit at Bay Area Community Health.
