A Message from Our CEO Dr. Zettie D. Page III
As I reflect on another remarkable year at Bay Area Community Health (BACH), I am struck less by the numbers than by the lives they represent. Every patient who walked through our doors, every family that entrusted us with their care, and every member of our team who answered that call, remind me why this work has mattered so deeply throughout my career.
This past year, BACH served more than 69,000 patients, delivering over 357,000 visits across our medical, dental, behavioral health, vision, and enabling services. Behind each of those encounters is a child receiving preventive care, a senior managing a chronic illness, a family finding stability through care coordination, or an individual who finally found a place where they felt seen, heard, and welcomed.
One of the most encouraging aspects of this year's report is the continued growth of our comprehensive integrated model of care. Our dental program provided care to more than 36,600 patients through nearly 98,000 visits. Our vision services reached more than 14,000 patients. Equally important, our enabling services team (case managers, community health workers, health educators, outreach staff, and patient navigators) served more than 52,000 individuals, ensuring that healthcare extends beyond the exam room or operatory and into the everyday realities of live our patients face.
These accomplishments are possible because of ordinary people like you and me performing extraordinary work. I have had the privilege of working alongside providers, support and administrative staff both paid and volunteers, and board members whose dedication continues to inspire me to do more. They show up with compassion, skills, and a commitment to our mission every single day.
Our work has never been about simply treating illness. It has always been about strengthening families, communities, reducing barriers, and creating opportunities for healthier lives. As the needs of our region continue to evolve, so too does BACH. We are investing in a system of integrated care, expanding access to behavioral health, enhancing services for children and families, and embracing innovative initiatives while remaining grounded in our core FQHC safety net values that have guided this organization from the beginning.
I have been fortunate to witness the growth of BACH out of both Foothill and Tricity community health centers into one of California's leading providers of integrated, culturally responsive care that continues to be adaptive and flexible healthcare partners. That progress belongs to everyone who has contributed to this journey—our patients, staff, partners, supporters, and community leaders. Together, we have built something that is larger than any one individual: an organization defined by service, resilience, and hope.
Thank you for believing in our mission and for walking beside us. The future of Bay Area Community Health is bright because it rests in the hands of people who care about the communities we are privileged to serve. It has been one of the greatest honors of my professional life to be part of this remarkable organization, and I look forward with confidence to all that lies ahead.

Whole-Person Care for Healthier Communities
At Bay Area Community Health, we believe everyone deserves access to quality healthcare. Through our integrated care model, we provide medical, dental, vision, behavioral health, and support services that address the needs of the whole person.
In 2025, we served 69,263 patients across southern Alameda and Santa Clara Counties through our clinics, school-based health centers, and mobile health services. We proudly care for one of California's most diverse communities, including low-income families, agricultural workers, individuals experiencing homelessness, people living with HIV/AIDS, and other historically underserved populations—regardless of their ability to pay.
By combining comprehensive clinical care with care coordination, health education, outreach, and social support services, we help patients overcome barriers to care, navigate the healthcare system, and access the resources they need to achieve better health.

Bay Area Community Health is a nonprofit, Federally Qualified Health Center (FQHC) dedicated to delivering high-quality, patient-centered healthcare in a welcoming, respectful, and inclusive environment. Our mission is to improve the health and quality of life of the communities we serve by expanding access to comprehensive healthcare services, advancing prevention and wellness, and supporting the management of chronic conditions.

Stories of Healing, Hope, and Community
Every patient at Bay Area Community Health has a unique story. Some come to us for medical care, while others need support, guidance, or someone they can trust. We believe healthcare is about more than treating illness. It is about helping people overcome challenges, improve their health, and reach their goals. These stories highlight the care, support, and hope our patients have found along their healthcare journey.
Finding a Healthcare Home
When Farida moved to the United States with her family, navigating an unfamiliar healthcare system felt overwhelming. A Bay Area Community Health community health worker helped her enroll in health coverage and connect with a provider who spoke her language. She received preventive care and treatment for health concerns she had delayed addressing. Today, Farida and her children receive regular care and have a trusted healthcare home they can rely on.
Care Beyond the Diagnosis
After losing his job, Jorge struggled to manage both his health and the stress of financial uncertainty. At Bay Area Community Health, he received coordinated support from his medical provider, behavioral health team, and patient navigators. Together, they helped him address his healthcare needs, connect to resources, and regain stability. Today, Jorge is back at work and better able to manage his health and well-being.
A Path Toward Hope
After a major life change, Michael began struggling with depression and found himself pulling away from family, friends, and daily activities. Through Bay Area Community Health's behavioral health program, he connected with a therapist who provided support and practical tools to help him move forward. Over time, Michael rebuilt his confidence and found a renewed sense of purpose. Today, he volunteers in his community and encourages others to seek help when they need it.

Primary Care Services
Bay Area Community Health provides checkups, preventive care, health screenings, vaccinations, and treatment for common health concerns. We also offer women's health services, including prenatal care, family planning, and cancer screenings. In 2025, our care teams supported more than 9,000 patients with hypertension and over 7,000 patients living with diabetes.

Caring for Children

Supporting Student Well-Being
School-based health centers bring healthcare directly to students where they spend most of their day. Street Medicine and Substance Use Services: Expanding Access to Care
Bay Area Community Health's Street Medicine and Substance Use Services teams help people get the care and support they need. The Street Medicine team brings healthcare directly to people experiencing homelessness. The Substance Use Services team provides treatment and recovery support for people affected by substance use. Together, these programs connect patients to care, improve health, and help people build healthier and more stable lives.
