It is the policy of Bay Area Community Health to accept all patients for care regardless of their ability to pay for services. Bay Area Community Health offers a Sliding Fee Scale to all income eligible uninsured or under‐insured patients. This program allows qualifying patients to receive medical and dental care at a lower cost. Eligibility for the Sliding Fee Scale will be established by determining the household size and the annual household income. This information must be updated annually. Bay Area Community Health requires valid proof of income, household size, and completion of a Household Assessment when applying for the slide. If a patient chooses not to provide the required information, then he/she will not receive the discounted rate offered through the Sliding Fee Scale. However, because TCHC is a fiscally responsible, independent, not-for-profit community health center, we strive toward encouraging all patients who can afford payments to be responsible in this area, so that we can continue being a key member of our community’s vital health safety-net.
The specific patient payment responsibility at the time of visit depends on the patient’s insurance or special health programs. Some patients may have a co-payment, a service charge, a sliding discount, or a share of the cost, all of which are required at the time services are rendered. Lab and pharmacy charges may represent additional charges that may be billed separately, depending on the patient’s insurance/program. A billing statement may be sent to the patient if required payments are not paid at the time of visit.
Patients should always bring insurance cards, health program membership cards, and proper identification for every visit to BACH. If certain BACH services are not covered by the patient’s insurance, the patient may be eligible for a sliding fee discount.
