We aim to provide the highest-quality care and service to all of our patients. We want your experience at BACH to be as safe, pleasant and comfortable as possible.

When you call, our Member Services department will work with you to identify your eligibility for free or low cost medical services and/or insurance programs.
We will make an appointment with a doctor, a nurse practitioner or a physician’s assistant.
For your first appointment:
Birth Certificate- if asked to
Proof of your address- if applicable
Social Security Card- if applicable
Proof of your income
if you have dependents, you must bring proof of their income as well.
Note: If you do not have any income, you will need to bring a Letter of Support from whoever you are living with.
The Letter of Support should say what kind of support they are giving to you (a place to stay, food, etc.) and have their name and phone number on it.
BACH's Sliding Fee Scale Program
Bay Area Community Health serves all patients regardless of ability to pay. Ability to pay is determined by household income and family size and is applied to all patients, including those with insurance.
